Requirement Checklist

  Application
  Resume
  Three reference letters (May not be from family members)
  Copy of Social Security Card
  Copy of Valid Driver's License
  Copy of Valid Vehicle Insurance
  Negative TB test or Chest X-Ray (Must submit original)
  CPR Card (Must submit signed original cards from American Heart Association or Red Cross)
  First Aid Certification (Must submit original)
  FBI Criminal Background Check (Must submit original)
  State FULL Criminal Background Check ( Must submit original)
  County Criminal Background Check (From any county where you have lived in the last three years. Must submit original)
  Copy of Diploma, GED or Proof of Education
  Training Requirement provided by LEL Home Services, LLC

If you are providing services to someone on the A&D or TBI waiver you must also provide:
Proof of Physical by a Doctor Indicating No Health Issues/Communicable Disease

It is your responsibility as an employee to insure that LEL has current, non-expired documents on file at all times. ANY missing or expired file requirement will result in the Direct Support Professional becoming ineligible to provide services until the requirement is met or updated.
